plan fee schedule
The plan fee schedule is the amount that Dentist's will charge you when services are rendered. These charges have already been negotiated with our dentists which are 25% to 60% of what the dentist would normally charge you based on a national fee profiling service. The plan fee schedule is based on the dentist's geographic location and does vary by location.
